We see her in KH1 when Sora goes into a flashback of Kairi's past, when she was a little girl back at Radient Garden.

 

I doubt she died, but she may have become a heartless when Malifecent took over and Kairi was sent to Destiny Islands.

@joby, actually Nomura stated that there are no deaths in KH. The only reason we see deaths with Disney characters was because of the Disney movies. That's the only excuse as to why you see Disney characters "die".

I read an Ansem Report that Xehanort (the apprentice) lost his heart from the experiment.. and since Xehanort was at Radiant Garden and so is the original Organization XIII (lea, braig, etc.) There might be an incident where all of them lost their heart becoming heartless and affected Kairi's Grandmother and some of the residents of Radiant garden from Xehanort's experiment of failure?
