Because Sora killed off Ansem and Xemnas (Xehanort's Heartless and Nobody), Master Xehanort regained his original body. This is explained in Re:coded.

Isn't it confirmed by now that the Unknown from BBS was in fact Young Xehanort? I'm not sure what you're asking...

I believe it's the opposite. After all, most people do call Xehanort's Heartless "Ansem" and the real Ansem "DiZ", so referring to him as Ansem is easier than having to say "Xehanort's Heartless" all the time and everybody will still understand who they're talking about.

That's not a glitch, when you enter Reality Shift everything happening stops, it happens always and will all Reality Shifts.

Sora's Special Portal 4 at La Cité can give: Noble Fantasy, Dulcet Fantasy, Lofty Fantasy.
