How difficult does the time limit make the game?

I've been thinking about buying it, but the thought of playing under a constant time limit seems really harsh. I'd constantly be wondering how much time I have left, and couldn't get myself to bother with the side quests.

So I wanna hear if the time limit is actually as bad as I think it is. How easy is it to run out of time?
