Xehanort took Ansem the Wise's name and used it to write his own reports. So because Xehanort as a somebody used the name Ansem, his Heartless and Nobody did to.

Why would you need Heartless' to summon Kingdom Hearts?

To your first question, then JTD95 has already answered it.

Second Question: There are many ways to make Kingdom Hearts. Collect the 7 princesses of light, destroy Heartless (Emblems) with a Keyblade.

I also read in a Xehanort report:

That if you complete the X-blade you also make Kingdom Hearts, because they live side by side.

The Kingdom Heart we see in the Keyblade Graveyard was probably created during the Keyblade war. Master Xehanort just removed the skies.

kingdom hearts was always full until the heartless came along after that u need to collect hearts so it would appear and the answer to the ansem one is what JTD95 said

Terranort eventually took over Ansem's research and decided to call himself Ansem because of that. In KH2, Sora looks at a portrait of Terranort and recognises Ansem.

Um, ever notice that there where heartless...oh wait, I believe you are talking about the Emblem Heartless, my mistake, it might be possible.

He's the Heartless of Apprentice Xehanort, who stole Ansem's name.

There are two types of Kingdom Hearts. The true KH, like the one in BBS, and the artificial one created from gathering hearts, like the one the Organization was creating.

With that said, since all Xehanort did was part the clouds, it can be assumed that KH has always been in the Keyblade Graveyard.

You don't make KH by collecting the PoH's. If you collect all seven hearts of pure light, you'll be able to forge a Keyblade of the heart (like the one Rikunort used in KH1), and create a path towards the true KH.

Is there some confusion between creating and forging a path to? Cuz I'm getting that impression. Completing the X-blade will create a path towards the true heart of all worlds. This is something you can't create.

Clouds, not the sky.

How exactly does Heartless affect KH when they've always existed? You only need to collect hearts to create an artificial KH. Forging a path to the true KH is done through other means.
