It was just Xehanort. Ansem the Wise has always been just a Somebody. He has never lost his heart, though he was sent into the darkness twice. Once by his apprentices and once by the machine he created to absorb KH and foil Xemnas' plans.

It's in the Ansem reports in KH2. There's a reason why these reports are in the game, not just to torture you by reading something so long lol.

Technically, I think Axel says it, but basically Axel says he's interested in why Sora kept his personality even though he was a heartless, and that Ansem SoD was the only other person to do it.

Okay, that's why I couldn't remember it.

If I remember correctly, the first time he was sent to the Realm of Nothingness and his will and himself almost disappeared, but he kept himself going by the thought of getting revenge. Truthfully, it wouldn't surprise me if he did become a heartless later on.
