Here is some notes I took down while attempting to understand the origins of Org XIII and some of their motives.. Wrote last November and updated today.
Xehanort, AtW, and 5 other apprentices work on the mysteries of the heart, probably during KH1, in the RG Castle.
Secret Ansem report 2 from KH2 says:
''That is when I discovered the "Ansem's Reports."
Though they bore my name, the only one I had written was number 0.(1 in USA) ''
So the remaining reports are a log of Xehanorts story, although in the Secret Ansem Reports of KH2 more is revealed, which AtW did actually write.
Xehanort expressed more interests in things in reports 9-12. Xehanort seems to not want a keyblade wielder to lock Radiant Gardens Heart. It's confusing. He seems to want access to RG'S heart and see it for himself to answer some of his questions. He looks like he wants the keyblade for himself, as it holds phenomenal power, and he says he talked about it with Mickey for hours(AtW did, not Xehanort). So he wants to use Kairi to lead him to a keyblade wielder(Sora). He may remember parts of his past as MX and Terra, and the keyblade still fascinates him.
A world's wall collapses because of the appearance of the Heartless, but finding a world's door takes time. And robbing a world of its heart is a similar case.
If the door has been closed by the Keyblade, one would probably be unable to reach that world's heart again. Before the Keyblade wielder appears in this world, I must take measures to do something.
Supposing that there is a close relationship between the Princesses(Kairi) and the Keyblade(Sora), it seems likely that they will resonate with each other...
I have chosen a special girl(Kairi). I do not know if she possesses the power of the Princesses, but there is a chance, and this is an experiment. She may lead me to the place where the one holding the key is...
I shall send her off to the ocean of other worlds.''
Meanwhile, according to SAR 2, AtW build a lab under the Castle. Then the 6 apprentices started abducting citizens to conduct experiments on. It tells us Xehanort and the apprentices strayed past the door to the heart of RG but were lost to the darkness, thus creating 6 of the members of Organisation XIII, aswell as 6 corresponding Heartless', which according to Nomura probabl were nothing more than a bunch of shadows(minus Ansem SoD).

''My path is set. I shall seek out the wielder of the Keyblade, and the princesses. My body is too frail for such a journey, but I must do this. I will cast it off and plunge into the depths of darkness.''
By Ansem Report 11( in japan) and 12(in the USA), Xehanort says:
''I have transcended to an existence of only the heart. I should have come back as a Heartless, but there is no sign of such a transformation.''
So he became a heartless, using Ansems name, becoming Ansem SoD. He wants to find a keyblade wielder, but thinks his body is too weak(Ironic imo, as MX posessed Terra in the first place thinking his body was too weak). Therefore he gives into the darkness, and so Ansem SoD and Xemnas are born. He claims he is a special Heartless, retaining a human form and past memories. I would say that Axel and Saix were captured for experiments as the themselves were residents of RG. .All apprentices were reborn as heartless and as nobodies Roxas was found by Xemnas. This was the basis for Organisation XIII.
AtW was banished to the realm of nothingness(realm inbetween) by the traitors. It was here that AtW wrote his secret reports.. He was very spiteful and wanted revenge on his apprentices. He realizes what the traitors are doing,After this, it seems they attacked abducting citizens, making heartless from Ansem's machine and generally causing havoc.Like Riku, he learned to control his darkness and created a dark corridor to get back to the realm of light.He said this in SAR 6:
To deceive Xehanort and my apprentices, I first used my power to change form before returning to the realm of light.
To what form he took is not specified, but it may be that wierd costume with the bandage mask that he is referring to. He also gave himself a new name - DiZ. (Darkness in Zero)
He next found Twilight Town, a quite village between the RoL and the RoD, to which he set up his base in the abandoned mansion, carrying out research and wondering what happens to the body and soul after darkness takes the heart. The answer was nobodies. He soon discovered that Organisation XIII had been assembled, with his traitors at it's core, who were currently residing in Castle Oblivion, so he followed them. He discovers that Mickey and Sora had protected the RoL from any more heartless invasions and that Ansem SoD had been vanquished. But he says Sora defeated some of the traitors before he could, and rescued Namine, who had been held captive by the Organisation. They were using her to carry out experiments on memories.He and Namine brought Sora back to Twilight Town, to repair the memories he had lost in CO. DiZ stated that any world in the realm in between light and dark is unstable, this is why corridors of darkness appear in twilight town, Co, Traverse Town etc.Riku arrives on the scene( they met in CO previously, DiZ disgiused himself as Ansem,Riku and mickey see through his guise, DiZ is saddened that he cannot properly talk to Mickey.DiZ gives them Organisation Cloaks.DiZ gives Riku a card that will draw out Ansem, and although Riku wins, a part of Ansem still lingers inside him.) and says he will give his all for Sora.DiZ realises that Sora's memories are slow to return, and realises that his nobody is roaming one of the worlds, and without this nobody, Sora cannot wake up.
DiZ sends Riku to find him,his name is Roxas. Why is DiZ even helping Sora? He wants to use Sora to eliminate Organisation XIII for him, that is why it is indespensable that he should wake up. He wants Riku to bring back Roxas so he can convert him into data and insert him into Sora.Roxas was born in Twilight Town and DiZ knows the organisation will come for him. Riku succeeds in beating Roxas, but had to unleash the fragments of Ansem SoD in his heart to do it.DiZ is moved by how Riku gave up his appearance to save Sora, and slowly begins to lose his lust for revenge.Even so, DiZ put Roxas into the simulated Twilight Town until Sora's memories had been mended entirely. He suceeds, Roxas is inserted back into Sora. Sora wakes up and Roxas lives on inside him. Roxas was previousl being used alongside Xion to collect hearts to complete KH. It is strange how the organisations motives were to become whole and own a heart of their own. According to KH3D (I think)
the real purpose was for them to become 13 Xehanort's 13 darkness'. these figures would clash with 7 lights. the outcome is the formation of an X-blade. It is implied(or said) that Xehanort tells us that his previous attempt (Ven vs Vanitas) was hasty and he was careless. Ofc x-blade opens KH, in turn, control of all worlds. Xemnas was the only member who knew abou t the organisations true role.
