The location you're missing is Symphony of Sorcery with Riku, in the first area once you enter the musical score.

My bad, I assumed Riku went through the same path as Sora. (which he doesn't as I just got there) Fixed.

Lord Kyroo (or more commonly known as it's japanese name, Frog Prince) is a optional boss that appears throughout the game. You can find Lord Kyroo before you beat the game, and it's minions it spawns are a good way to grind up EXP. I haven't beaten the game yet (or found him in every world) so this is a WiP. He also does reappear in past spots, so don't worry if you don't defeat him after visiting all three spots. Don't forget: Lord Kyroo's HP does not reset, so the battle should be easy. You get the recipe for creating a Spirit version of him for your party as well as a HP increase and new drop bonuses.
