There's two parallel versions of the same world in DDD because Riku is in Sora's dreams and interacting in the same world, but a different imagining of it. It doesn't really have to make much sense since it's in the dream realm and is really just an excuse to explain why Sora and Riku don't interact in gameplay. Joshua's explanation for it in Traverse Town is a little bit more complicated than it really had to be. The journal's importance comes from the fact that even before Coded, it had apparently achieved some level of intelligence sufficient enough to basically add words to itself through Data Namine, even before she was created through Mickey's computer, to send a message to the Disney characters through the book: 'Their hurting will be mended when you return to end it.' Maleficent's interest comes from the fact that the journal, in its digitized state, holds data versions of the worlds that the journal has 'memory' of, even without the written words on the pages. This would obviously remind her of the Tome, as according to legend, it has the power to create entire worlds, as well as tell the future. Hopefully that makes more sense.
