Wouldn't we already have it confirmed? Or when do they normally announce when a HD version release?

Well they announced 2.5 in October of 2013 and it didn't come out until a year later in Japan so I guess if they where planing on releasing an HD version of DDD on consoles they would've done it by now but then again in 1.5 and 2.5 they had to make HD cinematic of Days and Re:coded so that's probably why it took so long, with DDD they will only have to work on one game and they would have to change a few things so maybe they are waiting until it is closer to being finished before they announce anything, but again who knows if they even plan on releasing DDD on consoles or not.  But going by what the credits on 2.5 showed it is possible that they plan to do something with it at some point maybe.

They can announce it 3 months in advance of release. It would likely be digital-only.

I feel the most likely scenario is it's announced some time during the first half of this year and gets a November or December release ( at least in Japan)

Well typically after the credits tease, it would wait until the next big event that Square is attending, like E3 or D23, something big enough for them to have a lot of attention focused on them. And like Sora96 said, they could potentially announce it a mere few months before it's planned release date, depending on how far into production or completion it is and what format it is on (digital or retail). So if there are plans for it, and I'm sure there are, we'll likely hear about it some time this year.

Or how cool would it be if they planned on releasing DDD along with a KH3 demo? Kind of like what they did with type-0 and Final Fantasy 15.

Not really, even if it was finished tomorrow they would still release it at the end of the year. Right now Type-0 is out, before it was KH2.5. It wouldn't be wise to release them so close to each other. It's just bussiness.

Because it's just a remaster. They do it all the time. Yes, but PS4 and Xbox One require digital versions for ALL games. So they would have to work something out with Utada because otherwise: no dice.
