Recently, I think it was my inability to remember my Shotgun existed in Last of Us Multiplayer. I was in many positions to use it effectively but I forgot I even had it and persisted to use my pistol. The best? Probably when I played FF13 and had to fight Dahaka a couple years back, bought a ton of Rune Bracelets and use my gil to go towards upgrading them to Witch'sBracelets, and then checked the shop and noticed I could have bought them all for cheaper. Still failed pretty horribly. My friend was over the next day, I had to do the dishes, so I explained the mechanics and such, and I come back in to check after like ten minutes and he beat the boss on his first firetrucking try.

I've seen a guy start up FFVII, get an hour and a half into his run, at the endgame, but he lost ten seconds and reset it immediately. It always happens with speedrunners, but the farthest I've seen, is about two hours into Silent Hill 4: The Room, one of my favorite speedrunners ran into a game-breaking bug that hung the game.
