Semi Difficult- Hero Mode, collecting Chain Mail. Insane- Hero Mode, Collecting Heart Pieces. Impossible- Hero Mode, 3 Heart Run. Impossibly Impossible- Hero Mode. 3 Heart Run. Play Blindfolded. These probably aren't even that hard until late game.
