He is nothing like that post-transformation - when the game actually gets going.

It's pretty much from its facial structure and characteristics, him having purple lips doesn't help much either. Its more of him having a feminine appearance, than being effeminated, unless I have missed something in the recent years in the Cecil discussion. Anyway here is some artwork and Cecil in the After Years: Though the game does say he was named Cecil, because of him having a very similar appearance to his mother Cecilia.
