So I was wondering since I was re-playing KH and CoM cause of the HD collection and I was wondering.. 

1. How did Sora and the others end up in those green fields at the beginning of CoM?

2. when did Goofy and Donald get new clothes??

The secret ending of KH shows them wandering through the fields.

Clothes... er.....  DON'T QUESTION IT!

Speaking of which, why was Mickey only wearing his trademark pants when the others saw him in the Realm of Darkness?

At "The End of the World" after Ansem had been defeated, and Kingdom Hearts had vanished, there had been a path behind the door that lead to who knows where? I would like to guess that at the end of all worlds, was the Land of Departure, and here is my reasoning:

1.) The Land of Departure was very grassy and had multiple gardens, and since Sora and Company was on their way to Castle Oblivion (Land of Departure) this is why the world looked barren.

 

As for why Donald and Goofy had new clothes is actually because Nomura worked on Kingdom Hearts backwards, which is why they are in their "Trademark" costumes, and so is Mickey at the end of the game. Nomura was asked this question in a interview back when the game first came out, after I post the comment I will see if I can find the article on kh13.com. I hope this helps!

It shows Sora walking in them at night for like two seconds in the Deep Dive secret ending, but that's about it.

There's at least a one or two week split as Roxas' first couple weeks in Days are the end of KH1, from the point Sora became a Heartless, to when Sora ends up in Castle Oblivion. So I'd say there is a small split between KH1 and CoM.

He was only set to appear in one scene in the game, so they hadn't designed an outfit for him.

Which worlds did Pluto travel when he had that letter?

 

Nomua: I can say that he appeared in worlds that do not appear in the game…. well, other than the current worlds, there are other worlds that the Heartless erased. The End of the World did not exist at first and is a world that is made up of erased worlds put together. Also, there is the place where the door to Kingdom Hearts appears. Originally, that was another world(Land of Departure), so that means that Sora and the others are now residing in that world which returned to what it once was.
