So while I was working (because making up theories is the best thing I can do while packing books) I thought about this weird theory which kind of makes sense if you hear me out.
Aqua will probably die at the end of Kingdom Hearts 0.2 Birth By Sleep: A Fragmentary Passage.
Please hear me out.
As we all know Aqua is currently in the Realm of Darkness and as she progresses she probably gets through the Worlds which fell to darkness during Kingdom Hearts 1.
However, it's pointed out that this is the prologue to Kingdom Hearts 3 which means that Fragmentary Passage takes place after Aqua meets Ansem the Wise.
And before you say, "It's 0.2 so it takes place before she meets Ansem!"
I totally disagree with you because 0.5 takes place during the moment Aqua starts her journey through RoD.
That could either mean that 0.2 takes place right at the moment Aqua enters the Realm of Darkness since Aqua, during the Last Episode ending, she says: "How long have I been down here?"
Implying that she had probably been traversing the RoD for quite some time before we see her.
However, since the game is called Prologue to Kingdom Hearts 3 we just have to assume that the game is the missing 0.2 from 2.8, which would mean the game takes place after the events of DDD.
So, Aqua decides to leave Ansem do, whatever he was doing, and find her way out of the Realm of Darkness, back to the Realm of Light.
Aqua is now at the darkest parts of Rod and the Heartless have grown really powerful, making it seem like she is still Level 1. But she has acquired a few new skills.
She can now Tranform her Keyblade and use Attraction Flow.
That would simply explain the new game mechanics.
So anyway, she keeps travelling until she finally gets really close to the exit.
However, a large number of powerful Heartless surround her.
She gets overpowered and at her last moments she apologizes to Ven for not being able to be there to wake him up.
BUT she figures out a way that will help someone, from the Realm of Light, to figure out the way to Ven's whereabouts.
She passes on her Keyblade along with the name of Keyblade Master to the first person to find it.
Aqua, sadly, fades away but her Keyblade, Master's Defender makes it out and in the end finds its way to Destiny Islands where Sora finds it (as shown in the Kingdom Hearts 3 PS4 trailer).
Curious about it, Sora decided to consult Master Yen Sid who then tells him that in order to find one of the Seven Guardians of Light he needs to travel the worlds and open pathways that will lead to Castle Oblivion.
Aqua may not be there to find the way to the Chamber of Waking but Sora now has her will, which has been passed on along with her Keyblade.
With that Sora will be able to solve the mystery behind Castle Oblivion and return Ven's heart right where it belongs.
I think this makes enough sense.
I don't know about you but seeing Aqua travel alongside Sora, or the two meeting each other, seems implausible, in my opinion.
The Master's Defender couldn't have just teleported to Destiny Islands on its own so it stands to reason that Aqua was behind it.
So either she died and passed it on like I said or she remains trapped in the RoD and Mickey goes save her but without a weapon to defend herself Aqua is as good as dead.
There is also a chance Mickey tries to save her during the events of Fragmentary Passage but fails in the end and Aqua trusts Mickey to get her Keyblade to the Realm of Light along with the secret to the mystery of Castle Oblivion. So again she sacrifices herself in order to send Mickey out of the RoD and right on Destiny Islands.
The theory could also be supported by the title: A Fragmentary Passage.

So with this theory, I connected Kingdom Hearts Final Prologue with Kingdom Hearts 3, gave you a reason for Sora to travel the worlds again and explained why Master's Defender was on Destiny Islands without its Wielder.
Now if you ask me, I HATE this theory.
Aqua is my most favorite character and I don't want her to die. She already suffered so much, she at least deserves a reunion with her friends.
If this turns out to be true I will cry way too much.
