I'm pretty sure the eyes aren't exactly a mistake. If you look closely, you can see that Ven's eyebrows are actually the color of Vanitas's. Meaning the Venitas in the 0.2 opening is an actual fusion of the two characters, with both having influence. Ven: Hair, face, eyes. Van: Eyebrows, outfit, personality, metal jaw thingy.

So instead of seeing the Kamehameha you saw the Kamehaha

And those five minutes took 25 episodes

Venitas having blue eyes isn't a mistake. If you look at the close-up of his face closely, you'll see that he also has black eyebrows. It's meant to show the internal struggle between Ventus and Vanitas. The blond hair and blue eyes are Ven, and the dark suit, metal jaw piece, and black eyebrows are Vanitas.

Actually I think the castle in the 0.2 intro is the castle shown before Disney films, and the orbs of darkness circling it and essentially shutting the lights symbolizes the Disney worlds falling to darkness.
