I have a headcannon that it's a part of Quadratum that broke apart when Yozora was facing the Gigas in the Toy Box intro. Luxord's somebody was caught in the blast which sent him to the realm in between where Xemnas found him years later and a world to call their own

The woman that Yozora lost ended up in the Final World as the Nameless Star

Dress habits of the KH cast

Sora: Tries to look presentable but rarely goes beyond. Puts extra effort into hair care.
Kairi: Similar to Sora but tries to look a little nicer for friends and family. Will wear light makeup when the situation is appropriate.
Riku: Less particular than Sora but still attempts to look decent. Mostly wears what is clean and convenient. Hasn’t bought new clothes in ages.
Ventus: Minimally concerned with appearance. Does put effort into his hair however.
Terra: Also minimally concerned with appearance. Works out often to maintain physique however.
Aqua: Immaculate dresser. Tries to look as professional as possible at all times. Will not leave home without thoroughly checking hair and makeup.
Axel: THERE IS NO THE FASHION. ONLY HAIR.
Roxas: [see Ventus]
Xion: Not overly concerned with appearance but does try to look nicer for her friends. Has no skill in makeup but would like to learn.

Its not. Its smack in the middle of the Realm Between. Its literally in the mid point between Light and Dark realms
