It can be at any point from any game that you have been through, despite what type of goal/objective you may have been doing in that game. Feel free to list multiple ones if you fell like it.

 

Super Mario RPG Legend of the Seven Stars - First fight with Culex(was totally underleveled and prepared when I first fought him), First fight of 3rd round of facing Jinx (more so that move Silver Bullet)

 

KHBBS - First handful of rounds with Myserious Figure/Unknown

 

Super Smash Bros & Mario Kart Series - I'm sure anyone that has played their share of these games had plenty of wth/wtf moments playing these games. (too numerous for me to count)
