I've thought about creating my own video game for along over a decade now.

Pretty much my character is someone that just struggles with depression and anxiety and the only people she interacts with on a daily basis are her own figments.

Most of them tend to be jerks.

How would I start to make my own video game much less my own plot since it would need a beginning, middle and end?

My advice is to sit down and start documenting stuff. Start with a one-pager, basically an elevator pitch, have down the age range, platforms planned, a brief narrative synopsis, gameplay structure, etc. If you look up game design one pager or game design document (GDD), that might be a good plavce to start
